I'd guess that's pretty much how it went in the days prior to WW2, and hasn't changed much at all since WW2.
You get this clearly in the book "With the Old Breed" by Sledge. He joined 1st MarDiv after the 'canal and was at Peleliu as a Pvt. Did not sound like any kind of fun, that one. That's a superbly written book IMHO.
